Finding and accessing social services and community resources can be challenging for stroke survivors and their care partners.  The purpose of this systematic review was to identify and review interventions that aimed to increase access and use of such services and resources after stroke.

Our findings suggest that there is insufficient evidence to support any clinical practice recommendations towards any one particular approach. Findings did highlight key considerations in the design of interventions to support the needs of this population group.
